Once we receive your information, we will be contact you to arrange a 5-10 minute phone call to review all details ahead of the appointment. If your dog is currently under veterinary care, the vet's permission will need to be obtained by email before the first appointment can go ahead. Please include your vet's details on the form and we will contact them requesting permission.
